Community Care

Community Care is funded by the Federal Government. There are a range of individually planned and coordinated care packages designed to assist aged members of the community to remain living in their own homes, rather than moving to a residential aged care facility.

How Can You Apply ?

To be eligible for Commuinty Care you must first have an assessment completed by the Aged Care Assessment Team (known as an ACAT assessment), which can be organised through your local doctor, healthcare professional, or hospital.

You can also contact the Aged Care Australia Information Line on 1800 200 422 for information about the Aged Care Assessment Team nearest to you.

ACAT assessments are free and can be carried out in your own home, or at the ACAT offices, which are often located within a hospital.

If it is deemed that a Community Care package is appropriate for your needs, you will then be referred to an RSL Care WA Community Care Coordinator.

How Much Does It Cost ?

In most cases, you will be charged a fee for your Community Care package. The fees are subsidised by the Australian Government.

If you are on the basic pension, fees cannot be more than 17.5% of your pension.

If you are on a higher income, you may be asked to pay an additional fee of no more than 50% of any income above the maximum pension rate.

How Is The Package Arranged ?

If a Community Care package is able to be allocated to you, a range of services will be tailored to meet your individual needs, as assessed by the Aged Care Assessment Team.

You will be able to discuss the options available for meeting your care needs with your RSL Care WA Community Care Coordinator, and a Care Plan will be agreed upon.  You will receive a copy of your individual Care Plan for your records, setting out the services you will receive.

The two main Community Care packages that are available through RSL Care WA are:

CACP - Community Aged Care Package, which is also referred to as "Low Care"

EACH - Extended Aged Care at Home, which is also referred to as "High Care"

How Is It Monitored ?

The Federal Government has very strict and specific Community Aged Care Standards, which providers, such as RSL Care WA, are legally required to meet, ensuring our Community Care recipients only ever receive individual care of the highest quality.
